We do suggest that you take the medical form along to your doctor for to be completed and signed before the first class.

2. Download the above forms (3).
3. Get a scuba medical done, and have the medical form signed by your doctor.
4. Make a deposit of fifty percent, non-refundable, but transferable deposit prior to the start of the courses.
